HLHS 202 - Community Resources


PREREQUISITES: Demonstrated competency through appropriate assessment or earning a grade of “C” or better in ENGL 093 - Introduction to College Writing  and ENGL 083 - Reading Strategies for College  or ENGL 095 - Integrated Reading and Writing , or ENGL 075 - Co-Requisite Integrated Reading & Writing

Introduction to social service record keeping and community resources. Emphasis will be given to universal documents found in most agencies, as well as record content, format, sequence and structure; overview of common community agencies and typical services provided by each. Emphasis is on identifying and discussing the uses and applications of community resources in supporting patients and their families. Students will learn and simulate techniques for interacting with patients and their families, and will examine collaborative strategies for interdisciplinary healthcare team efforts.

MAJOR COURSE LEARNING OBJECTIVES: Upon successful completion of this course, the student will be expected to:

  1. Integrate knowledge from other courses to gain an understanding of individuals, families, groups and communities.
  2. Identify and utilize skills required for effective interaction between and among the patient, family, health care team, and the community.
  3. Incorporate healthcare practices which demonstrate respect for ethnic identity and socio-cultural practices of clients in the community.
  4. Assist patients to exercise their rights to select, participate in and evaluate health care.
  5. Recognize the socio-political and economic issues in the community which affect the client’s health and healthcare, and serve as an advocate when appropriate.
  6. Initiate, evaluate, and adapt teaching strategies to insure appropriateness for individual patients, families, and groups.
  7. Collaborate with other health care providers and patients/families to provide and evaluate health care.
  8. Demonstrate knowledge of community resources and services available to support both patients and their families.
  9. Recognize and describe the ethical/legal issues surrounding privacy, confidentiality, and fair access to community resource records.
  10. Demonstrate knowledge of general eligibility criteria for common community resource agencies.
  11. Categorize and classify common agencies/programs according to primary needs addressed and populations served.
  12. Demonstrate appropriate empathy for the difficulties and issues experienced by populations served by community resource systems, as well as employees of the system.
  13. Recognize, identify, and acknowledge their own personal areas of bias and identify how that may impact their effectiveness in providing care and objectivity in written documentation.
